Afriqiyah Airways Flight 771 is in trouble, and the pilots don’t even know how quickly things will turn deadly. It’s the end of a long overnight flight; the crew are tired, fatigued and lost in a thick fog as they desperately try to find the runway at Tripoli airport, but they can’t find it and are about to crash.
Breaking free of the fog the plane hits the ground just seconds later, a matter of metres short from the safety of a runway.
A series of mistakes, misinterpretations, distractions, poor planning and incomplete information have left the aircraft in a state of ruin. The accident claiming the lives of over 100 people.
For investigators, what’s puzzling is that this accident presents many familiar tones. It’s a type of plane crash that the aviation sphere has spent decades trying to prevent, yet here we are.
If we were to piece this mystery back together, what exactly would we find? Well getting to the heart of this accident not only requires an understanding of the chain of events that brought this plane its critical moment, but also an understand of the sensory human dangers that can threaten the lives of anyone who boards an airplane.

Being Dutch, I still remember it vividly. Can you imagine being a 9 year old sole survivor of a plane crash that killed your entire family? His grandparents took care of him after the accident, but he's generally stayed out of the media. I really hope he's doing well.

Well, as a Libyan, I would elaborate on something so important I noticed within our dialect, which can be important to understand the captain's incompetence, at 36:56 there's a huge mistranslation here, شفته does not used as just "you see?" which can be ambiguous, it literally means "Did you see it?", and for any sane person in that situation, I think that the captain must know that the question is about the runway.

I just wanted to come in and say this: I've believed that you are an excellent documentarian from the first video I watched, but last night I realized what an excellent educator you are. My boyfriend and I were discussing the recent UPS crash and he was asking questions I actually had factual answers for. And all of them came from watching your content. So, thank you for teaching me.

What puzzles me is the third pilot in the jump seat doesn't seem to have added anything to the unfolding disaster. As a third pair of eyes and ears, he could so easily have pointed out erroes in procedures. Truly CRM was seriously awry with this airline. Also the fact the captain was flying for two different airlies at the same time could have only added conflicting procedures to the mess.

As an HC-130-H Navigator in the not so distant past, I must say, you did a great job presenting the complexities of the terminal environment. Especially the approach, and the 10 or so minutes leading up to it. There are so many things that go into an approach. They can go South really quickly when small mistake are made, and the holes in the cheese start lining up more quickly.

Wow, imagine hearing "Terrain, pull up!" and your first reaction as a captain is to take control without informing the pilot flying and immediately steer the plane into the ground.
That part astounded me more than anything else. The lack of communication and not following proper procedures is complacency, but making the decision to take control at a critical moment without informing the pilot flying and pushing the stick forward when you've just been warned you are already in a dive straight into the ground is beyond incompetence.
The first officer could have easily saved the plane and avoided a crash if the captain hadn't interfered. He killed all those people and is 100% responsible for their deaths. What an absolute moron.

While I’ve never suffered a somatogravic illusion, I have gotten the ‘leans’ once before. This is when you come out of a continuous turn and fly straight and level, but your body feels like the new straight and level attitude is a steep turn. I had this happen to me in perfect day VFR conditions and still had to fight my body to keep the wings truly level. I cannot imagine the level of disorientation at night, especially if both parties are feeling the same thing!

I was on that flight number a few weeks previously. I'd booked it because it was by far the cheapest return ticket from London-Joburg. I remember being very impressed with the experience. Tripoli Airport terminal was a dump, but the staff were friendly and a spanking new and very modern looking one was under construction. In fact I was so impressed I flew with them again not long afterwards, this time from London-Dhaka. The Tripoli-Dhaka leg & v.v. was also on the brand new A330, so if there were 3 of them it's very likely that I actually flew on the accident aircraft at some stage. My blood ran pretty cold when I heard the news.
