It looks like a simple icing accident. It wasn't. Behind it was an airline where doing the right thing got you threatened, defects went on Post-it notes instead of the logbook, and every safety net had quietly been removed. What seems like a simple crash goes far deeper than you'd expect.

This is the full story of Air Ontario 1363.

My grandfather was on this flight and survived! My father has been a huge fan of your channel for years and he's thrilled you were able to make a video about the flight his dad was on. Thank you!

It sems to me that this company made their own holes in the cheese.

@SkiMyMountain 3 months ago

What is really amazing is that over 40 people survived that fiery crash into the woods!

Great to read that Judge Mushansky is still very much alive at 97 years of age. I can imagine the hard work he put into this saved numerous lives.

@Nurichiri 3 months ago

Not only were the Swiss cheese holes lining up, more were actively being created.

Usually when I hear "get-there-itis" there's a part of me that thinks 'if only they stepped back for a second and thought it through,' but in this video I just feel so bad for the captain and his crew. I can't imagine what it must feel like to have your own company actively trying to find new ways to screw you over, especially when it involves the safety of the people you're flying.

@PatrickRiendeau-m3v 3 months ago

The fact that judge Moshansky was also experienced pilot contributed to the quality of the report.

100% holes, zero cheese. This awful outcome was inevitable, given the compounding of so many unsafe elements, practices and culture. Thanks for the comprehensive report, Captain Petter.

As someone that has worked aircraft maintenance, the yellow Post-It notes hurts my soul.

...there's "holes lining up"
and then there's "poking more holes - with a jackhammer"

You've been doing these for so long I think it's time you tackle one of the most divisive and political crashes in recent years, the crash of Tupolev Tu-154 at Smolensk with Polish President on board. There is so much misinformation and conspiracy theories it would be nice to have a complete, informative video from you guys.

Happy to hot fuel the bird without the firecrew on hand, but not able to de-ice with running engines?
Sheesh...

I'm not often shocked but 1 week of training for a dispatcher is actually insane.

@eugenestein8530 3 months ago

It feels like a story made up for the exam, "find every single thing done wrong" rather than actual case

Hi Mentour Pilot,

I don't know if you'll ever read this message, but I wanted to take a moment to express my gratitude.

I've been watching your channel for the past six years. My dream is to become a pilot and one day fly the A350-1000. Your videos have played a huge role in inspiring that dream and helping me learn more about aviation. Through your accident analyses and explanations, I've learned valuable lessons from others' mistakes and gained a deeper appreciation for safety in aviation.

I'm incredibly thankful for the time and effort you put into creating such informative content. Your explanations are always clear, detailed, and easy to understand, and the way you present each story makes it both engaging and fascinating to watch.

Thank you for all the knowledge and inspiration you've shared over the years. Keep up the amazing work.

5:29 It's worth to mention that Virgil P. Moshansky (born 1928, now at 97!) was not a random judge - as a private pilot he used the plane he owned to fly between his office and courtrooms across the province, so his aviation knowledge and expertise was instrumental in leading the investigation. After publishing the Dryden crash report he spent further three years advising Transport Canada on the implementation of his commission's 191 recommendations for regulatory reform. His work has been recognized by Transport Canada, which presented him with Canadian aviation safety award in 1995. Then he continued to consult internationally on aviation matters. In 2007 he was elected as a fellow of the U.K. Royal Aeronautical Society. // (BTW, at 1:23 - a COBUS 3000 in 1989? Details matter!)

This isn’t the Swiss cheese model, this is the hole with a microscopic bit of cheese in the area.

@Fetzenkrieger 3 months ago

That were not some holes in the Swiss cheese lining up. This was an army of mice -- a lot of them in manager suits and ties -- eating the whole thing.
