German Report Points to Pilot MISTAKE on Boeing 787!

Germany's BFU has seized the brake computers from a Vietnam Airlines Boeing 787-9 after brake inputs were recorded on its August 15, 2026 takeoff from Munich. The jet struck its tail and lifted off from the grass. No cause has been determined.

They should add a feature that automatically prevents the over writing of voice recordings once any alarm/caution/warning occurs.

There needs to be an automated, loud audible warning if a pilot applies the brakes while the throttles are set for takeoff. How is this not already a thing? 280 people almost died.

What good is a 2 hour loop? It would be constantly overwritten during any typical 787 flight.

Great update. What impresses me is that the 787 was able to take off with the brakes locked up, even while heavily loaded. That puts the power of these engines into perspective. Thank goodness there were no injuries. It's a sad way to end a career though.

In what world would you lower the main landing gear for a fly by check suspecting MLG damage and then raise that same MLG back up again once it’s all down and locked you leave it down because next time may not be the same outcome

To quote Juan Browne, "Keep your feet off the damn brakes!" In fact, if braking is applied during the takeoff roll, the cockpit warning message should be "KEEP YOUR FEET OFF THE DAMN BRAKES!!!" No need to be subtle about it.

Excellent report. Clear, concise, extremely well explained for the non pilot aviation enthusiast such as myself. I especially appreciated some of the video clips of things like the flaps extended after fully extended position with the gap showing; another noted aviation safety analyst.I've seen explaineded this but did not illustrate it - so that's where your higher production values really excel. Kudos on a very well done video. And congratulations on your book, sounds like one I'l be very interested in reading when it comes out!
