I am still just astounded at two things, regarding "care".
1.  None of the prescribers did any labwork.  None looked at possible hormonal imbalance as a pausible contributing factor.
2. Not a single one the the "clinicians", asked Lindsay fir a release so the prescribers could consult with one another. They all seemed to know that she was being treated by other care givers, all of them relied on Lindsay to keep them all informed.   So not ONE of them was directing or managing her care, coordinating it.  I think McClain or one of the facilities reached out to Jolatta and never received a call back. That is the only time, i heard ANY of them reach out to the others or even suggested that Lindsay  have a single point person to coordinate.

For me, it speaks volumes that the prosecution couldn't find EVEN ONE expert in Postpartum Depression/Anxiety and Psychosis who was willing to testify for them.

When Red calls for a sidebar you know he's fired up 🔥

I’m sorry but if my spouse tells me he’s having thoughts about hurting my kids I would not leave my kids alone with him under any circumstances!! I would ask my in laws to move in with us or have a nanny to help out

I went through a lot of the same mental health issues as Lindsay Clancy. Extreme depression and anxiety, severe insomnia, obsessive/intrustive thoughts and thought broadcasting. I also thought I could hear people far away talking about me but I couldn’t fully understand what they were saying, among other delusions. The psychiatrist I saw thought my psychosis meant I was maybe developing schizophrenia. I was on a lot of the same meds as Lindsay. These issues lasted for a very very long time. I’m honestly surprised I made it out on the other side. One thing I will say, is that I only experienced one instance of clear auditory hallucination, that I was able to recognize as a hallucination. It was in the middle of the night while laying in bed. A single dog bark so extremely loud that it scared me, and actually hurt my head. It felt like it came from inside my head but also outside of my head. Never happened again. What would otherwise be a very innocuous sound was absolutely terrifying. I had other olfactory hallucinations which I only knew were hallucinations because I would ask other people if they could smell it, and it was always the same  chemical smell that no one else could smell. I believe her because I experienced much of the exact same things myself. I didn’t report all of my specific thoughts, beliefs or feelings to anyone because I was so scared that I was losing my mind and didn’t want anyone to know the true extent, and I didn’t want to scare my friends and family more than they already were. It took me years to get the courage to finally start cognitive behavioural therapy, and even then I never gave every single specific thought, feeling or experience I went through.

Lord have mercy!!! If Mr FBI would cut out all of the "uuhhhh's" and "uuummmmmm's" out of his answers we may get through this sometime this year

Can we all agree that Beach Blanket Bingo sounds WAY more fun than Beach Body work out videos 😂

The prosecution deliberately chose expert witnesses who do not believe PPD is a real mental illness and I think red is trying to show that without coming right out and saying it

I still will never understand why OCD manifestation into psychosis has never been considered in this trial

"She did lots of checking of the distance" .... didnt she literally just look up how far it was? I do that anytime I send my husband on an errand lol

I feel like I need to listen to this witness on 2x speed

Can the lip smacking just stop??

Red at the start: so you wrote a paper in the 1970s about post partum yes?
Kirk: ye that right
Red at the end: just a minor correction, that paper you wrote wasn't actually about post partum was it? it was schizophrenia

I hate that I can tell how hard they are fighting to not say something that would be beneficial to the defense instead of just answering the question.

Anyone who thinks postpartum ends in 4 weeks is ignorant!  You aren't even done bleeding at that point! Clearly men came up with that garbage.

Homeboy with his back to the judge staring at the ceiling willing it to be over is too funny.

Backtracked on the crushed pills only after she mentioned them. Surely everyone in that courtroom noticed that? Completely lying on the stand.

This is much better for Reddington than “they rejected her because it wasn’t postpartum.” Heilbrun just had to retreat to a more qualified account:

Women & Infants did not conclude she was fine. They concluded she had generalized anxiety and depressive disorder, with the depressive picture not further specified because of adverse drug effects, and they thought a general mental-health setting fit better than their postpartum day program.

Because the earlier shorthand:

“They didn’t accept her because it wasn’t postpartum”

sounds much more definitive than the record Heilbrun is now describing. What the record apparently says is closer to:

We see significant anxiety/depression, but medication effects are complicating the clinical picture, so this particular postpartum program isn’t the right placement.

That is a very different evidentiary proposition.

And Reddington got a potentially important phrase into the testimony:
“due to adverse drug effects.”

I would be careful with his word “overmedicated,” though. Heilbrun did not quite concede, “Yes, the doctors overmedicated her.” He conceded that Women & Infants documented medication complications/adverse drug effects affecting the diagnostic formulation. That supports medication-related clinical concern; whether it proves negligent overmedication is another question.

But look at the chronology this produces:

She goes specifically to a specialist postpartum program because she is badly deteriorating. They evaluate her. They identify anxiety, depression, passive suicidality and medication-related complications. They do not accept her into that particular program. They offer other treatment pathways. She returns to her existing outpatient prescriber the next day.

That does not equal:

“Postpartum specialists evaluated her and ruled out serious postpartum mental illness.”

And that's important because Heilbrun had been leaning on the fact that Women & Infants didn't admit her as some evidence against the postpartum formulation.

After this exchange, I think the most defensible characterization becomes:

Women & Infants thought her presentation did not fit their postpartum day-program criteria and believed general psychiatric treatment was more appropriate, while also documenting significant anxiety/depression and adverse medication effects.

Much less dramatic.

The other thing Reddington is trying to establish is a handoff problem. He asks whether Women & Infants actually contacted Jollotta about the medication concerns. Heilbrun says he didn't see that in the records.

If that holds up, the interesting system question becomes:

Did anyone take ownership of reconciling the medications and the rapidly worsening psychiatric picture?

Because from the testimony we've heard, she seems to move through:

Tufts → emergency care → Jollotta/other NP → Women & Infants → back to Tufts/Jollotta → MGH → McLean → outpatient again,

with different diagnoses and medication strategies appearing along the way.

That doesn't prove malpractice or causation. But it does reinforce the picture of a patient moving through multiple points of care while the formulation remains unsettled.

“The records indicated diagnosis of generalized anxiety disorder and depressive disorder without further specification … due to adverse drug effects.”

That's another reminder that even the contemporaneous clinicians were apparently saying, in effect:

We cannot cleanly tell what psychiatric syndrome we're looking at because the medication picture is muddying it.

That makes retrospective certainty 3 years later harder, not easier.

It doesn't mean Heilbrun can't reach a diagnosis. He has years of subsequent records unavailable to those clinicians.

But when the Commonwealth experts say:

“There really wasn't evidence of psychosis; this was bipolar II/MDD…”

the defense can fairly answer:

The people actually seeing her during the deterioration weren't even working from a clean clinical picture. They were documenting medication effects, shifting diagnoses, profound insomnia, suicidality, dissociation and worsening symptoms.

That is exactly why I keep coming back to epistemic humility in this case.

One thing I would put a red pin beside for later is:

WOMEN & INFANTS — exact discharge diagnosis + exact wording on “adverse drug effects” + exact reason for declining postpartum program + whether they contacted Jollotta/Tufts.

Because Reddington is paraphrasing aggressively, Heilbrun is paraphrasing from his own report, and those distinctions are too important for us to rely on either man's courtroom shorthand.

The viewer numbers these trial livestreams are doing are insane. It seems clear that at baseline this trial is shifting the public conversation about women's mental health. I hope we'll continue this conversation long after the verdict is in, regardless of what the verdict is.
