The song is an opening to an anime called "Tokyo Ghoul". In this anime there two kind of human species. Humans and Ghouls. In the events of the series the protagonist Kaneki Ken get turn into a Ghoul. The lyrics are about his transformations and losing himself to a new self...a new identiy.

Her vocal agility is just insane.

It is the theme song of an anime "Tokyo Ghoul" about the fear and despair of a man who loses his humanity and becomes a monster (Eating human to survive) after being transplanted with the organs of a monster in an accident.

Yeeeees, she is the closest to a stage musical performer. Usually, the studio version of Ado's songs are much more 'refined' and 'contained' while her live versions are way more intense, emotional and overall 'unhinged'.

Since some fans have said, “She looks tired,” I was happy to see that you all understood that was just part of Ado’s performance style.
At her concerts in Japan, Ado sings inside a cage during the first half and steps out of the cage for the second half, but she doesn’t step out of the cage when performing overseas.
Unfortunately, this is likely because there are a certain number of people overseas who don’t follow the rule prohibiting photography inside the venue.

Yeah she's very much a performer and not just a singer. She sort of "acts" through the songs she performs, gives that necessary emotion etc.

This is actually a well done cover song

I know enough of the context to cry my eyes out every time I hear her vocals.

I think if you want to watch another live performance from her from this same live show that definitely shows how intentional she was with her breathiness at parts in this song, you should check out her song Show! She does always have the cage for her live performances because it's an added layer of protection for her to help conceal herself, but you'll also see her leave the cage frequently in Show as well, so she's not ALWAYS in it :)

Without a face, you can 'close your eyes' and feel/hear the music.

Her vocal cords are forged from steel

Seeing her in concert is transformative. I have likened her to creating a theatrical performance. She pours her emotions into her songs. She is an incredible performer.
