Excellent presentation, thank you Sara.  I am waiting for my post DIEP Flap fat grafting in a month.   I can relate to everything she said…expanders were rough - I was a DD, went down to a C cup after initial excision, reduction and lift.  The expanders were tough because I wanted the same volume I had prior.  My Plastic surgeon said this was a way better option than implants.  I also had my ovaries removed due to hyperplasia, but kept uterus.  I was told that the liposuction would be done on my sides due to front abdominal incision dehiscence (which was a horrible and slow healing) which, 4 weeks out, is still open 4 inches wide, but no longer deep (wet- to-dry saline gauzes).

How much size does the fat grafting add? I just had a DIEP Flap with double mastectomy two weeks ago. I will have the revision surgery in December, if all goes as planned. I have one side that needs nipple reconstruction as well. Not sure what to expect on that end. I felt more pain than this patient in the chest area. I did immediate reconstruction though.

Is this possible for detransitioned (ex-FTM) women? I got a double mastectomy 5 years ago and I’m really set on getting diep flap with nipple reconstruction if it’s possible. My nipples are completely flat and my chest is completely board flat

At what stage does nipple reconstruction happen? At the end?
