This game control system consists of a pair of large sliding levers, red for Player 1 and blue for Player 2. The levers travel horizontally along a single shared curved rail across the front of the cabinet.
Because the levers physically collide and cannot pass one another, the control system allows two players to directly push and jostle each other during competitive play. A start button is also provided.

Players control cartoon roller-skating characters, represented by red and blue figures, by sliding the cabinet levers left and right. Since both levers share the same rail and physically block each other, two-player games encourage players to push against one another, with the physical movement of the controls directly affecting their characters on screen.

The game consists of short, randomly selected mini-games drawn from a pool of twenty-five, each with its own time limit, scoring rules, or life system.
The mini-games are divided broadly into coin-collection and survival types. Coin-collection games require players to gather coins or other objects while avoiding hazards; large coins are worth ten small coins, while collisions can cause collected coins to be lost. Survival games use either one or three lives.

In single-player play, the unused lever can be locked at one end of the rail. Difficulty dynamically adjusts between Easy, Normal, and Hard according to the player's performance.

It begins with a short practice segment in which the player collects coins. The number of coins collected determines the difficulty of the first stage.
The game then randomly selects four stages, although operator settings can increase this to six, from the full pool of twenty-five mini-games. These are followed by a final stage. Completing a stage awards a Success Medal if the required quota is reached or at least one life remains. Failure occurs when the required quota is missed or all lives are lost.

Performance on each stage affects the difficulty of the following stage. The final stage is selected and scaled according to the number of successful stages completed, with a greater number of successes resulting in a more difficult final challenge.

Clearing the final stage awards a trophy and ends the credit. Five mini-games—Rocket Panic, Electric Panic, Hammer Panic, Space Panic, and Jumping Panic—appear exclusively as single-player final stages at certain difficulty levels. The remaining twenty mini-games can appear in any stage position.

Two-player versus mode also begins with the shared practice coin-collection segment, which establishes the initial difficulty for both players.
The game then randomly selects five mini-games, with operator settings able to increase the number to seven. Both players compete simultaneously in the same stages.

In coin-collection stages, the player with the higher coin total wins the round. In life-based stages, the player with more remaining lives wins; if both players lose all their lives, the player who survives for the longer time wins. Equal results produce a draw.
The player who wins the greater number of rounds becomes the overall victor. Continues are available and add additional rounds when used, normally three rounds or five under certain operator settings, to determine the winner of the match.

Panic King is a single-player mode accessed by holding the start button while inserting a credit, or by holding the start button while the cabinet is configured for free play.
The mode presents one fixed mini-game for each calendar month at a difficulty higher than the hardest level available in the standard modes. The attempt continues until the player fails. Each player receives two attempts, with the better score being entered into a monthly ranking table.

Panic King relies on the cabinet's internal clock to determine the active month, and the month cannot be freely selected through the test mode.
