In trepang2, there's a nuclear warhead in a silo at the maon headquarters you stay at, and if you melee the controls too many times it overloads, you hear the director sigh in disappointment over the radio, and the nuke detonates, ending the game instantly.

I was dumbfounded when Dishonored actually had a response for trying to jump in the tub with her. I almost died laughing.

Baldur's Gate 3, the "i can fix her" death, recruiting Minthara after changing her mind on being evil, then trying to bow chica wow wow her and she kisses you, and.....you die. She has the kiss of death that first needs to be dispelled but conveniently she forgot. I did do this one myself. Astarion called me an idiot and I didn't listen.

It's kind of refreshing to see you all talking more about older games on a list then usual

How about saga frontier 1? Early in one of the stories you meet the final boss early. He implores you not to try to fight him. You can fight him and the boss is ridiculously hard, but you can win. If you do, the boss reminds you that he told you not to fight him, then breaks the fourth wall and says your game is over.

Several games in rhe Disgaea franchise have an alternate ending if you Game Over against certain bosses.

Also Nier Automata gives you one of the 26 possible endings if you happen to die during the game's opening mission.

Super Mario Kart has a cool music track that you can only hear if you finish a Grand Prix outside of the top three and thus fail to win a trophy. I didn't know about this until I saw an FAQ about it on GameFAQs, because I never finished a Grand Prix cup off the podium. I did, however, once get to see the equivalent scene in the sequel, Mario Kart 64, while playing with my dad when I was a kid.

in Nier Automata, there's a game over where if you try to stare up skirts of either 2B or the Commandant, well 2B just covers herself and looks disapprovingly, but eventually the Commandant kicks your butt away and then beheads you. I swear, I didn't discover this myself, it was a google search, and none of you can prove otherwise

It's not an accident if you intentionally jump onto the Midas statue's hand out of curiosity 😂

Mass Effect 2. Morinth's Malevolent Mature Mating Murder. The player is warned repeatedly not to do Naughty Things with the Ardat-Yakshi. Ignore those warnings at your peril.

in almost every FromSoft Dark Souls and Soulslike game, there is one game over that everyone should know better than to fall for: Trusting Patches. Nuff said.
