A graduate entry medical student so this is really important for me to see

I appreciate the effort in focus on nativity reservation but the main problem i believe is the fact the NHS as a healthcare company can't seem to retain its workforce longterm. You see most of the workforce especially doctors and nurses burning out after a few years once, when the honeymoon fascination with an initial nhs job is over, then searching for better overseas options. There is a major leakage happening and pouring more water without focusing on the leakage is not gonna bring any change apart from creating great headlines and pumping more money.

Tbh the need to prioritise uk doctors . Our merit is based on their training and uk medical training is not one of the best let’s be honest . We aren’t competitive like the US Grads. Canada does it . we need to start doing that soon . Uk is a popular country to move to so we need to also have training positions where we call home .
Maybe for consultants it can be a free for all, but whilst training the uk should continue training their graduates

The graduate entry medicine course in scotland (ScotGEM) allows students to opt into a bursary of £4k per year in exchange for a commitment to work in the Scottish NHS for one year per year they claim the bursary (i.e claim bursary for all 4 years, work in the Scottish NHS for 4 years after graduating, this includes F1/F2). I think this is a pretty good way of incentivising doctors to stay in the UK NHS honestly, though the bursary could do with being scaled up with inflation.

Every year a record crisis. Record waits, with huge uncounted 'secret' lists, and even getting onto a list is a wait. Maintenance backlog is HUGE: Just the urgent parts are greater than the whole annual repair budget (always raided to pay for deficits). Political solution: Bed cuts, 130,000 gapped posts and transfers of hospital work to community where there're another 160,000 gapped posts (not including GP shortfall). Since 1948 NHS reforms every~18 months, fails each time the same way.

As a community nurse, the move away from hospitals is worrying. Psychosis is already struggling for beds for people who NEED to be there due to risk to themselves and others. Throwing more money at community services won’t open a new ward for patients who cannot be managed safely at home.

I disagree with the idea of a mandated service period for dentists and doctors.

But similar to what you’re saying 3 years of “tie in” for doctors would just be hidden with in existing training programmes - so pointless.

they could do a similar thing with the military, i.e. if you go to uni here you have to be in the NHS for at least 5 years or 10 years etc.

Unemployment awaits. Replacement by AI in the public sector. A scramble for places in private healthcare where face to face appointments will stay the norm.

Giving priority to UK graduates on specialist training does not make any sense. It is fair to give priority to them for foundation training. But when it comes to specialist training it should be based on merit which is the current basis of recruitment. This is clear cut discrimination. It is completely against the equality and diversity.
