Speaking strictly from an Emergency Management EOC viewpoint, whether it is a full EOC activation or one of our command/scene trailers going to a scene, we still use command boards, NIMS forms, etc. regardless of how small or large the incident is. Just as you were saying, we never know when the small scene will expand tremendously. If it does, we’re ready and don’t have to play catch up.

Accountability is hugely lacking when it comes to LODD. If we have a near miss, we will tear that incident apart and go through it with a fine-toothed comb. But when someone dies, it seems like that dynamic completely changes. Whether it's not wanting to speak ill of the dead firefighter who may have made a mistake or pointing fingers at the command structure that contributed to it. We need to start having the hard conversations. Hell, I've seen officers get posthumously promoted because it was the "feel-good" thing to do even though that officer's decisions directly contributed to the LODDs.
