Pretty sure that the alternative for Izuku to keep living, instead of a "famous" doctor, would be Eri. Though he would be always seen as a guy between his 16 to 19, maybe Kyoka too, if she wanted to be with him as long as she can. Why 16 to 19? Because Izuku still wants to be a hero, and at the age of 15, even if he could have kept OFA, his body wouldn't be suitable for its full power, and around 16-19 he could be a hero just fine.
