In this video, I explain, with models, what hip prosthesis dislocation is. An important point: dislocation is not the prosthesis "coming loose from the bone" (the stem and cup remain fixed). It is the femoral head that escapes from inside the acetabulum. The causes are grouped into three: inadequate positioning of the prosthesis, loss of tension in the soft tissues (muscles, capsule, and loose ligaments), and impact from extreme movements.
First of all, a relief: considering all scenarios, dislocation is rare. A study of nearly 210,000 prostheses found an incidence of approximately 1.4% — and, with good choices, this number tends to be much lower. Next, I present the accessible preventative measures in any scenario: choosing an implant that replicates the anatomy, correct positioning, care for the soft tissues, patient education, and good physiotherapy. And the innovations that further reduce the risk in selected cases: direct anterior access (which preserves tendons and muscles), mini-stem, larger diameter heads, surface prosthesis, and dual-mobility prosthesis.
The final message is the same as always: the most effective measure to avoid any complication is the choice of professional and team.
