Medieval kings weren’t uniquely monstrous.
They were just less subtle.

There was a Portuguese king called Pedro, and he was a contemporary of Pedro the Cruel of Castile, he was actually his uncle. He married a Castilian noblewoman called Constanza Manuel, and he started an affair with one of her ladies in waiting, called Ines Castro, a Galician noblewoman. The events that transpired were the ones that Eleanor narrates here.
It is an easy mistake. Both men were called Pedro, both were considered Cruel or Just depending on who you ask, and they lived at the same time.
