Spencer needs a background.... It looks like he's sitting in a dark garage.

1:27:32 I looked quickly and had to do a double take because I thought the helmet behind the doc was a metal casting of Hulk Hogan’s head. Lol

I was very curious about the doctor's statement that epinephrine would "clamp down" both peripheral and coronary arteries and I found that this does not seem to be true as far as I am reading. Epinephrine seems to have a uniform dilatory effect on coronary arteries, not a constrictive one. I do understand that pacing is probably still the best option, but I am still relatively confused on why epinephrine is apparently such a terrible choice that shows they don't understand the physiology.

I wonder if reluctance to pace an “awake” patient wasn’t the main problem here? They recognized that he was sick enough to possibly arrest during the call (pads on, IO prepared, etc.) but in all of their explanations and reasoning as to why they chose a pressor over pacing they ONLY state the possible benefits of meds and NEVER explain the possible harm of pacing.
This patient clearly needed to be paced, and I suspect if you gave a similar scenario to this same crew on paper they would verbally state pacing as the appropriate intervention.
This sounds to me like trying to medically justify why they didn’t do the intervention that at some level they knew he needed, they just couldn’t pull the trigger. Either because they’ve never paced a patient before, never paced an awake patient before, etc.
1:15:00 - you nailed it!
I believe this entire scenario plays out like an emotional decision NOT to pace was made early, and all of the “pathophysiology” was considered after the fact as a way to justify that decision.
So I don’t think their misunderstanding of the cardiovascular system led them astray, but was a way to justify to themselves why they chose the meds.
I don’t think this was done consciously! But if on reflection you don’t realize that’s what happened you can’t use that lesson to push you through that wall of fear/uncertainty next time.

Great. Episode!!!!!!
Better living through lightning whenever needed.
If someone has idea to be discussed on RSI where can we give the suggestion?
